Form ADV
Form ADV is the registration and disclosure document every SEC-registered investment adviser must file and keep current. It describes who we are, how we do business, what we charge, and any conflicts of interest you should know about before working with us. We update it at least once a year.
Fortis Portfolio Solutions LLC, doing business as Fortis Private Wealth, files its Form ADV with the U.S. Securities and Exchange Commission. You can read every part of it on the SEC's public database:

What each part covers
- Part 1 — the firm's structure, ownership, affiliations, client base, assets under management, and any disciplinary history. The SEC uses this part to register and examine advisers.
- Part 2A (the brochure) — a plain-language description of our advisory services, fees, investment approach, risks, conflicts of interest, and the background of the people who manage the firm. This is the document we encourage every prospective client to read.
- Part 2B (the brochure supplement) — the education, experience, and disciplinary record of each person who gives investment advice to clients.
Requesting a copy
We provide the brochure and supplement to every client before or at the time an advisory agreement is signed, and annually thereafter if there are material changes. You may request a free copy at any time by emailing mer@fortisportfolio.com.
Our Form CRS, a two-page relationship summary written for retail investors, is also available on this site.
